Updated Medical History Revision

When we ask for an updated medical history we already have their dental history. (These questions should be removed) Why are you leaving your previous dentist is also irrelevant as they are already our patient. Insurance question should simply ask has there been any change(s) to your insurance information? Yes or No (Cells to add info should populate only if yes is checked). Same with address, simply ask has there been any changes to your address? Yes or No (Cells to add info should populate only if yes is checked). simply ask has there been any change to your insurance information? Yes or No (Cells should populate only if yes is checked). Our patients repeatedly ask us "Why are you asking this information if you already have it?" It just makes us look silly.
