Some fields should not be mandatory

Not everyone has a landline or work phone to provide. I understand possibly leaving the cell number field mandatory but not all three.

The medications questions on recall form does not provide a box to list details. This needs to be added. Also the updated medical history has not been corrected yet. Please remove the “who referred you” question as they are simply updating their medicals...
